How to Order a Replacement Certificate
 
1.  Fill in the form below with all possible information (Exact dates and times are greatly helpful and will speed up the process) and click the "Submit" button. You will then be presented with a page that you will be able to print out.
 
2.  Send the printed form and $20 (cash, cashiers check or money order) and a self-addressed stamped envelope to:
 
IF YOU SEND A PERSONAL CHECK, THE CERTIFICATE WILL BE HELD FOR THREE (3) WEEKS UNTIL THE CHECK CLEARS! TO HAVE THE CERTIFICATE SENT QUICKER, THEN YOU MUST SEND EITHER CASH, A CASHIERS CHECK OR A MONEY ORDER.
